Your State Parking Permit for People with Disabilities (PPPD) blue hang tag has either expired or will expire soon. The expiration date is listed on your permit and can also be found on the page to follow.

Any person who has been issued a disability parking permit who abuses any privilege, benefit, precedence, or consideration arising from possession of the permit, may have it revoked.

The following requirements must be met by the applicant (the person with a disability) to renew a State Parking Permit for People with Disabilities (PPPD).

Must own a New York State Issued Driver's License or New York State Issued Non-Driver's ID. Both are available from the New York State Department of Motor Vehicles (DMV). Call New York State DMV at 212-645-5550 or 718-966-6155 for more information.

Must be a New York City resident. Residency will be determined by address as indicated on New York State Issued Driver's License or New York State Issued Non-Driver's ID. The address as indicated on your New York State Issued Driver's License or Non-Driver's ID must be current. If your application is approved, your permit will be mailed to this address.

  1. On the application to follow, you will be required to supply general personal physician information.

  2. Once you submit your application, it will be reviewed for completeness.

  3. Once you have received your new permit, return expired permit to:

    We must receive the expired permit within 2 weeks or you will be ineligible to renew your permit in the future.

    NOTE: All information that you submit with this application and any subsequent medical information will be kept confidential and will only be shared with those involved in the permit process, to the extent permitted or required by law.
