  1. Complete the online application.
  2. On a blank sheet of paper, write your name and application number (the application number will appear on the submission confirmation page).
  3. Sign and date the paper. If you are submitting a never received permit application, the signed paper must be notarized.
  4. Mail the signed paper to us, together with all required documents (see chart below-required documents vary by permit type (City or State) and application type), to the following:

    Parking Permits for People with Disabilities (PPPD) Unit

New Permit Application verifying letter from school or employer (if non-NYC resident),

copy of vehicle registration card for all vehicles registered outside the State of New York (you are not required to mail vehicle registration card copies if you list no more than two vehicles, and both are registered within the State of New York; if you list 3 or more vehicles, you must mail vehicle registration card copies for all vehicles, regardless if the vehicle is registered within the State of New York or not),

medical history page completed by your personal physician (call us at (718) 433-3100 or TTY (718) 433-3111 to request a blank medical history page) or ask your personal physician to complete your medical history online!
Give your personal physician the e-Permits web address (http://nycserv.nyc.gov/ePermits/Wf/Welcome.jsp), your application number and, with your other personal information (name and date of birth), s/he can complete your medical history quickly and conveniently online.
Lost or Stolen Application police incident information slip,

copy of vehicle registration card for replacement vehicle (if vehicle registered outside the State of New York)
Never Received Application none
Important: signed paper must be notarized
Permanent Plate Change Application copy of vehicle registration card for all vehicles not listed on current permit (if vehicle registered outside the State of New York)
Renewal Application verifying letter from school or employer (if non-NYC resident),

copy of vehicle registration card for all vehicles registered outside the State of New York (you are not required to mail vehicle registration card copies if you list no more than two vehicles, and both are registered within the State of New York; if you list 3 or more vehicles, you must mail vehicle registration card copies for all vehicles, regardless if the vehicle is registered within the State of New York or not),

recertification page completed by your personal physician (if you are due for reassessment, this page has been enclosed with the renewal notification letter mailed to you.)
Important: if you did not receive a renewal notification letter call us at (718) 433-3100 or TTY (718) 433-3111 to request a blank recertification form or ask your personal physician to complete your medical history online!
Give your personal physician the e-Permits web address (http://nycserv.nyc.gov/ePermits/Wf/Welcome.jsp), your application number and, with your other personal information (name and date of birth), s/he can complete your medical history quickly and conveniently online.
